Our doctors and nurses are focused on building healing connections with each patient. They listen, ask follow-up questions, and build trust, so that when you leave our office, you feel heard, understood, and valued.

We know that your time is valuable and so we use efficient workflows that keep our clinic teams running on-time. This minimizes wait times and gets you back to feeling better in less time and with fewer missed hours.

While efficient workflows mean you move through our clinic effectively, we also prioritize face-to-face time with your physician so that there is adequate time to hear, understand, and address your medical concerns.

Available

In most cases, we can see you the day you call, unlike many clinics where appoint slots are weeks or months out. While we discourage walk-ins, you can usually get an appointment within 24 hours of your request, and typically, with your primary care provider.

Compassionate

We use compassionate care to get to know each patient on a human level. We pair you with your local care team at every appointment, where possible, so you see the care providers you know and trust and who have a real interest in your future well-being.

Convenient

Most of our clinics are on the employer's work-site - within walking distance from your desk. Additionally, lab draws, which typically require patients to visit an outside facility, can be done in a Health by Design clinic.

What healthcare services does the clinic provide?*

Will my employer have access to my medical information?
No, your Health by Design clinic strictly adheres to HIPAA guidelines. Also, all electronic medical records are maintained on a secure Health by Design network to ensure patient confidentiality.
What if I already have a PCP or Specialist?
No problem – consider the HBD clinic an extension of your care team. With your permission, the Health by Design medical team can coordinate care with your current doctor/specialist so you can receive care when needed.
Can HBD clinic physicians prescribe medication?
Yes, HBD physicians can prescribe medication to patients who use the clinic. You provide a preferred pharmacy for the prescription, then pick-up and pay for these prescriptions at your preferred pharmacy. At most HBD clinic locations, staff can dispense limited over-the-counter medication and prescription medication samples.
Do I need to schedule an appointment?
Yes, we strive to provide same-day or next-day appointments and to pair you with your dedicated healthcare team. Please call 866-412-6503 to schedule your appointment.
Where are your clinics located?
Most of our clinics are located on-site at your employer's place of business or in the vicinity of the employee or member population. Contact your human resources or benefits department for the location of your dedicated Health by Design clinic.
Will I have to pay or use insurance?
In most cases, there is no payment or copay required. Your employer provides the Health by Design primary care clinic at no cost as a benefit to you. Your employer selected Health by Design (HBD) because we utilize a relationship-based care model that is about more than physical health, but about inspiring passion for life with health and hope.

What if I need to cancel / reschedule?
We understand emergencies arise that require rescheduling appointments. At certain times of the year, we cannot guarantee availability for rescheduled exams. Please keep this in mind if you need to reschedule your appointment. If you must reschedule, please call our office at least 72 hours in advance to avoid a possible cancellation fee.
Do I need to fast?
The night before your exam, do not eat or drink anything after midnight, except for water. We encourage you to drink at least 16-20 ounces of water the morning of your exam.

If you have an afternoon appointment, please fast for a minimum of 6 hours before your appointment time.

We offer a variety of healthy snacks and beverages for you to enjoy after we draw your fasting lab work.
What should I wear?
Please wear, or bring with you, workout clothing and a comfortable pair of shoes with rubber soles for running. Shower facilities, towels, and personal products are available for you to freshen up upon completion of your exam.
How long will it take?
Your executive exam appointment typically takes 3 - 4 hours.

The follow-up results consultation, which occurs 3 - 4 weeks after your exam, takes 30 minutes to an hour.
What if I'm currently taking medications?
Please take all medications as prescribed. If you have any questions regarding the taking of medications, please call our office and speak with one of our Registered Nurses.

Please call your primary physician concerning any diabetic medication instructions on the day you are fasting.

Health & Nutritional Coaching


How do I enroll?
Contact your designated clinic and tell them you'd like to enroll in the Health & Nutritional Coaching program.
How long is the program?
The Health & Nutritional Coaching program typically runs 32-36 weeks and includes weekly appointments with your nurse educator.
What's the time commitment?
Participants are encouraged to attend weekly, 30-minute educational, and accountability sessions with a nurse educator. Appointments may also include biometric assessments, such as height, weight, blood pressure, pulse, waist circumference, and BMI.
What is included in the program?
There are two coaching tracks within the Health & Nutritional Coaching program: Weight Management and Tobacco Cessation. Patients can enroll in the track(s) that fit their needs. Each track includes:
  • Medical support - nurse consultations and physician visits when needed
  • A tailored plan to meet individual needs
  • Weekly individual counseling and accountability
  • Education to assure each patient has adequate knowledge to live a healthy life
  • Tools to promote long-term success
Do I need to schedule an appointment?
Yes, our Health & Nutritional Coaching programs are very popular and often have a waiting list. Please call your designated Health by Design clinic to enroll in the program and schedule your appointments.
