Evaluate and Improve Employee Wellness

Health by Design is committed to improving the lives of our patients through a preventative approach to health care. Early identification plays a key role in treating many health conditions. When you take advantage of our Corporate Health Assessments program, your employees are given the opportunity to take charge of their own health.

 

During a Corporate Health Assessment, each patient will experience:

 

  • Biometric assessment (height, weight, blood pressure, pulse, waist circumference and BMI)
  • Health risk assessment and lifestyle questionnaire
  • RN interventions for “critical issues”, as well as post-event follow-up
  • RN-delivered coaching to review assessment results and determine a course of action to help improve well being.

  • Comprehensive lab testing, including:
  • Diabetes screening
  • Liver testing
  • Lipid testing
  • Kidney function evaluation
  • Iron studies
  • Thyroid testing
  • Uric acid level testing
  • Prostate-Specific Antigen (PSA) as indicated

Our testing is second to none, but what really sets Health by Design's Corporate Health Assessments apart from traditional exams is our after-care. Patients are given the opportunity to discuss their Assessments results with a nurse educator, either by telephone or in person. Our professional and passionate clinical educators explain each component of the assessment results and provide recommendations for improvement.

Participants also receive a results booklet from Health by Design containing their laboratory results and an outline of each of the key points in the assessment. The booklet also includes historical trending of results that can be used as a catalytic resource patients can take to their primary care provider.

Early detection:

Between 9 and 17% of patients screened are determined to have a critical issue warranting immediate treatment to prevent emergency or acute problems. Left undiagnosed or untreated, these issues can pose a serious threat to your company’s workforce and productivity.


Preventative engagement:

Most diseases can be treated through lifestyle changes and patient education if identified early enough
